御参加皆様の情報は私たち主催者が確認し、ボートキャプテンと共有することが重要となります。
To ensure a safe marine leisure experience, we ask that you fill out the necessary paperwork in accordance with Okinawa Prefecture regulations.
It is important for us, the organizers, to verify the information of all participants and share it with the boat captain.
沖縄県警察本部水上安全対策係、第11管区海上保安本部の指導の下、参加の際に確認が必要となる書類は下記のものになります。

参加日当日の朝の皆様のコンディションを含めて確認/記載して頂きます。
マリンメニューに参加する皆様に1人1枚必ず記載して頂きます。個人情報他、緊急連絡先など重要事項の記載をお願いしております。
Fun Diving Participation Application Form / Confirmation Form (Waiver of Liability) English
All participants in the marine activities will be required to fill out one form each. Please include important information such as personal information and emergency contact details.
病歴診断書 / Medical History Certificate
ダイビング後の飛行機登場手順、キャンセルポリシー、病歴確認事項の再確認になります。当日朝は体調の自己診断、アルコール摂取状態の確認もさせて頂きます。
（We will review the procedures for boarding the plane after diving, cancellation policy, and medical history confirmation items. On the morning of the day, we will also check your physical condition and alcohol intake.）
