マリンレジャーを安全に行うため、沖縄県条例に則った書類の記載をお願いしております。
（To ensure a safe marine leisure experience, we ask that you fill out the necessary paperwork in accordance with Okinawa Prefecture regulations.）
沖縄県警察本部水上安全対策係、第11管区海上保安本部の指導の下、参加の際に確認が必要となる書類は下記のものになります。
（Under the guidance of the Okinawa Prefectural Police Headquarters Water Safety Division and the 11th Regional Coast Guard Headquarters, the following documents must be submitted to participate:）
活動をより安全に、円滑に進める為に出来る限り正確な情報を提供頂きますよう宜しくお願い致します。
（It’s not difficult, but in order to make our activities safer and smoother, we ask that you provide as accurate information as possible.）

下記の書類は参加日当日の朝の皆様のコンディションを含めて記載頂きます
ダイビング参加申込書 / Diving Participation Application Form
マリンメニューに参加する皆様には1人1枚必ず記載して頂きます。個人情報他、緊急連絡先など重要事項の記載をお願いしております。
（All participants in the marine activities will be required to fill out one form each. Please include important information such as personal information and emergency contact details.）
確認書（免責同意書）/ Confirmation (waiver of liability)
内容を確認頂き当日の注意事項、確認実行を理解して頂き署名を頂きます。未成年の参加者様は親権者の同意が必要となります。
（Please confirm the contents, understand the precautions and confirmation procedures for the day, and sign the document. Minor participants will need parental consent.）
病歴診断書 / Medical History Certificate
ダイビング後の飛行機登場手順、キャンセルポリシー、病歴確認事項の再確認になります。当日朝は体調の自己診断、アルコール摂取状態の確認もさせて頂きます。
（We will review the procedures for boarding the plane after diving, cancellation policy, and medical history confirmation items. On the morning of the day, we will also check your physical condition and alcohol intake.）
